What We Do
Community Health Workers (CHWs) connect community members to culturally appropriate health education and information, give social support and informal counseling, and advocate for individual and community health needs.
Advocacy
We empower community members to improve the health and social well-being of their community by identifying and resolving issues that increase health disparities. (i.e., transportation, lack of accessibility, food insecurity, etc.)
Care Navigation
We assist individuals with identifying and enrolling in health and human services. (i.e., WIC, SNAP, Medicaid, etc.)
Health Services
We partner with the community to coordinate mobile vaccination and testing clinics to minimize risk of vaccine preventable diseases.
Outreach & Education
We share information on health topics that will increase health knowledge and improve quality of life. (i.e., one-on-one health coaching, presentations, workshops, education tables, etc.)
