To make changes to your practice location address, registrant name, or drug schedules on your current South Carolina (DPH) controlled substances registration, follow the applicable instructions below. To make corresponding changes to your current DEA registration, follow the applicable instructions on the DEA’s website.

Change of Practice Location Address

If you are planning to move to a new practice location, you must notify DPH and DEA prior to your relocation date. To notify DPH, you must submit a completed Address Change Request form (D-1199) via email to bdc@dph.sc.gov or via fax to 803-896-0627, in order for an address change request to be processed by DPH.

Registrant Name Change

To make changes to your registrant name on your DPH controlled substances registration, you must notify DPH via email at bdc@dph.sc.gov or via fax at 803-896-0627 with your name change request and attach a copy of the marriage license, divorce decree, or court document that supports the change, in order for a registrant name change to be processed by DPH.

Drug Schedule Changes

To make changes to the drug schedules on your DPH controlled substances registration, you must submit a completed Schedule Change Request form (D1025) via email to bdc@dph.sc.gov or via fax to 803-896-0627. After your DPH controlled substances registration has been amended to reflect schedule changes, you will need to amend your DEA registration by going to the DEA’s website here. Guidance for how registered Physician Assistants (PAs) and Advanced Practice Registered Nurses (APRNs) request drug schedule changes on their DPH controlled substances registration is provided below.

Physician Assistants (PAs) may request to add drug schedules, including Schedule II narcotic substances and Schedule II nonnarcotic substances, to their existing DPH controlled substances registration. PAs must submit a copy of their Expanded Authorization to Prescribe Schedule Controlled Substances Approval Letter from the South Carolina Board of Medical Examiners with their completed Schedule Change Request form via email to bdc@dph.sc.gov or via fax to 803-896-0627, in order for a schedule change request to be processed by DPH.

Advanced Practice Registered Nurses (APRNs) may request to add drug schedules, including Schedule II narcotic substances and Schedule II nonnarcotic substances, to their existing DPH controlled substances registration. APRNs must submit a completed Schedule Change Request form via email to bdc@dph.sc.gov or via fax to 803-896-0627, in order for a schedule change request to be processed by DPH.
